1. Permission to Participate
I hereby give permission for my registered child/children to participate in soccer activities organized by Langwarrin Soccer Club.
2. Assumption of Risk
Parents and children acknowledge that soccer, including training and games, is a contact sport. They assume all risks associated with participation, including but not limited to falls, contact with other participants and equipment, weather effects, and other reasonable risks. Accordingly, parents and children hold Langwarrin Soccer Club harmless against all claims arising from any injury, loss, or damage suffered during authorized club soccer training or games.
3. Insurance Cover
All fully paid members are registered with Football Victoria (FV) and are covered by FV’s group insurance policy, providing basic insurance during FV competitions and club training. Parents and guardians are advised to consider obtaining private health insurance coverage. Langwarrin Soccer Club is not liable for any costs not covered by the FV Insurance Policy.
4. Emergency Treatment Authorization
I authorize Langwarrin Soccer Club to provide emergency treatment for any injury or illness to my child, as deemed necessary by qualified medical personnel.
5. Player Grading
For certain age groups, the club may grade players according to their abilities to ensure competitive balance among teams.
6. Communication Consent
Langwarrin Soccer Club may send emails (including marketing materials) to the supplied email address during registration.
7. Data Transfer to Third Parties
Parents and guardians consent to the club transferring personal data about themselves and/or their child, including names, email addresses, and mobile numbers, to carefully selected third parties providing services to the club. These third parties may contact you regarding the services they provide but will not use your details for other purposes.
8. Parental Assistance
Parents and guardians are expected to assist the Team Manager on training and match days, which may include tasks such as running the BBQ, setting up nets, and providing refreshments for players at halftime. A duty roster will be developed and distributed by the team manager.
9. Working with Children Check (WWC)
Adults working with children on behalf of Langwarrin Soccer Club must provide a copy of their Working With Children Check (WWC). Parents/guardians of members under 18 must ensure at least one adult with a WWC can fulfill duty roster responsibilities.
10. Membership Fees
Players will not be permitted to train or play unless full membership fees are paid. Langwarrin Soccer Club reserves the right to withdraw any player or team that has not paid its membership fee prior to the season commencement.
11. Refund Policy
Fees are generally non-refundable; however, requests may be considered in certain circumstances. All refund requests must be submitted in writing to treasurer@langwarrinsoccerclub.com, and a $50 admin fee may apply. No refunds will be issued once player FV registration is completed.
12. Conduct and Disciplinary Action
The club reserves the right to suspend or expel any player or member for failing to abide by the club’s rules and regulations and our code of conduct.
13. Contact Information
Contact details of parents/guardians of all players will be shared with the coach and team manager only.
14. Consent for Promotional Materials
By participating in Langwarrin Soccer Club programs, I consent to the club using photographic images in promotional materials unless specified otherwise in writing.
15. Application for Family Membership
I hereby apply for family membership in Langwarrin Soccer Club. I have read and understood the club’s rules and conduct regulations, and I agree to conduct myself in a manner that will not bring the club or the game of soccer into disrepute. I declare that the details provided are true and correct.
By agreeing to these terms and conditions, I acknowledge my understanding and acceptance.
